Yoğun Bakım Ünitelerinde Sık Karşılaşılan Etik Sorunlar

Öz

Bilim ve sağlık teknolojisindeki gelişmeler ölümle sonuçlanan birçok hastalıkta yaşam şansını arttıran, ileri teknolojiye dayalı bakım ve tedavi uygulamalarının sunumunu mümkün kılmaktadır. Bu durum bir yandan yaşam süresini uzatırken diğer yandan yaşam kalitesi, bakım maliyeti, yararsız bakım gibi etik tartışmaların ortaya çıkmasına neden olmaktadır. Yoğun bakım ünitelerinde çalışan hemşireler ve hekimler karar verme, aydınlatılmış onam, yaşam destek tedavilerinin kısıtlanması ya da sonlandırılması, kaynakların eşit dağılımı ve yaşamın kalitesinin sorgulanması gibi etik sorunlarla karşı karşıya kalmaktadır. Yoğun bakım ünitelerinde karşılaşılan etik durumların çözüme ulaştırmasında iyi bir ekip çalışması, etkin bir iletişim, ekip üyelerinin hasta hakları ve etik ilkelerle ilgili yeterli bilgiye sahip olmaları önemlidir. Makalede yoğun bakım ünitelerinde çalışan sağlık profesyonellerinin sıklıkla karşılaştıkları etik sorunlara, bu etik sorunların ortaya çıkmasını önlemeye yönelik yaklaşımlara ve çözüm önerilerine yer verilmiştir.

ABSTRACT

Advancements in science and health technology have made the supply of the care and treatments based on advanced technology increasing chance to live possible for many fatal diseases; which -on the one hand- provides longer life time and causes such ethical discussions as quality of life, cost of the care and futile care on the other hand. Nurses and doctors who work at the intensive care units face ethical problems such as decisions, informed consent, withholding or withdrawing of life sustaining treatments, equal distribution of the resources and questioning of the quality of life. It is highly important for the team members to have a successful team work, an effective communication, sufficient level of information about patient rights and ethical principles in order to solve ethical problems encountered at the intensive care units. The present article provided solution-offers to the ethical problems often faced by the health care professionals who worked at the intensive care units and to the ethical problems emphasized here and the approaches to prevent these problems.
